How to Register
If you live in our practice area, you will be asked to complete an application form and a health questionnaire. Also to provide evidence of identity (e.g. passport) and residence in our practice area (e.g. utility bill)
The practice does not discriminate against race, gender, social class, age religion, sexual orientation, appearance, disability or medical condition.
